Paid
leave for health screenings
Leave
of absence for donating blood
Employees may now
request paid leave for certain health screenings
In accordance with state law, the district will
provide employees paid leave time for the
purpose of obtaining breast and/or prostate
cancer screening as follows:
- Up to one
four-hour period annually (between 7/1 and
6/30) for female employees for the purpose
of breast cancer screening.
- Up to one
four-hour period annually (between 7/1 and
6/30) for male employees for the purpose of
breast cancer screening.
- Up to one
four-hour period annually (between 7/1 and
6/30) for male employees for the purpose of
prostate cancer screening.
Documentation:
If you intend to use time as outlined above, you
must complete a “Verification of Cancer
Screening” form at least one (1) week prior to
your appointment and submit it to your immediate
supervisor for his/her signature. After your
supervisor signs this form, it will be returned
to you. You must bring it to your appointment.
The provider’s office must sign the form. When
you return to work, please send the completed
form to the Office of Human Resources so the
absence can be properly recorded.
Please note: The Verification of
Cancer Screening form is a multi-part duplicate form, which
is not downloadable; it may be obtained from
your building principal or requested from the
Office of Human Resources.
Notes:
- The
leave time is not cumulative and expires at
the close of business on June 30.
- Travel
time is included in the time allotment.
- Absence
beyond the time allotment must be charged to
leave credits, sick, personal or vacation,
in that order to ensure that an employee
does not lose pay for any portion of the
day.
The school
district encourages you to have annual health
screenings and, as always, we would prefer that
you do so outside of work hours whenever
possible.

Leave
of Absence for blood donation granted to
employees
Effective July 7, 2008, all employees
who work at least 20 hours per week are allowed
three (3) hours of off-premises leave time in a
twelve month period to donate blood. The leave
time is considered unpaid leave; however,
available personal time may be used.
